REGISTRATION PROCEDURES
We have moved to an online registration system. Families can now upload copies of required documents. Once school is reopened families will be expected to provide original documents listed below in order to complete the online registration process.

Note: NO COURSE REQUESTS will be processed until ALL registration information is completed and returned.
CLASSES ARE ON A FIRST-COME, FIRST-SERVED BASIS
REQUIRED DOCUMENTATION FOR ALL ENTERING STUDENTS
Withdrawal papers from previous school (for current high school students)
Copy of transcript or last report card (for students who did not attend a middle school in the Berryessa School District)
Birth certificateUp-to-date immunization records, including If entering from outside Santa Clara County, records MUST include a TB Mantoux-PPD skin test completed within the last six months; if entering from outside California, records MUST include documentation of the varicella (chicken pox) vaccine or documentation of the disease. Also required: documentation showing the Tdap (whooping cough) vaccine has been administered.
Proof of residence in the Piedmont Hills' attendance area: Residency Status Verification Form (pdf) must be completed and the following documents provided:

PG&E bill, water bill, garbage bill, credit card bill, doctor bill, cable bill, health/car insurance statement, property tax statement, DMV car registration, paycheck stub, Registrar of Voters documentation, social service correspondence, bank statement
Official mail communications must be dated within 30 days of registration
We reserve the right to do home checks.
The 3 proofs of address are in addition to the residency verification document. Please refer to table to identify items you may need to bring.
FINAL TRANSCRIPTS
Final transcripts will be processed after June 15th, after the final grades are posted.
EDUCATION VERIFICATION
All verification requests must be in writing with the student's release. The request may be mailed or faxed. A request may be emailed- please use subject line "Education Verification-[Case # or Name]" to submit.
All students must go to the school in the attendance area where their parents reside.
If students do not live with parents, we require legal proof of guardianship. Parent or legal guardian must be present at the time of registration with a picture ID.

The Registrar's Office is located in the PHHS Main Office.
The Registrar processes requests for transcripts, duplicate diplomas, new enrollments, and withdrawals.
Please check processing times. Requests cannot be filled on the same day.
