Print and complete the documents below or pick up a work permit application in the counseling office.
Email completed forms to lagunar@esuhsd.org or bring to the counseling office during regular business hours.
Work permits WILL TAKE 5 BUSINESS DAYS to process once the completed application is received.
Students are required to come to the counseling office to pick up their work permit as they must sign the permit in person.
